Providence Care Center has exciting new opportunities for an Admissions Coordinator.

The Admissions Coordinator proactively recruits and assess/maintains census of residents for admission to the assigned area. The Admissions Coordinator provides service to older adults with the goal of offering the residents integrity, growth, and control over the qualities of their life.

Job Responsibilities:

  • Actively facilitates admission of new residents, readmits, and transfers.
  • Consults with the Director of Nursing and/or Nurse Managers for acceptability of prospective residents.
  • Conducts scheduled and walk-in tours of the facility for prospective residents, families, and/or guarantors.
  • Processes all admission paperwork (per state regulation and facility policy) for each admission, including computer input of all demographic and billing information.
  • Attends daily morning meeting and reports on planned admissions and census activity.
  • Communicates, as needed, with housekeeping to assure all rooms are ready for admissions.
  • Communicates, as needed, with maintenance regarding room upkeep and repair.
  • Satisfies need for communication of daily census activity.
  • Prepares daily census and resident location list for staff, administration, finance, and vendors (i.e. pharmacy, oxygen)
  • Updates computer program, at least daily, to facilitate accurate records for financial reimbursement.
  • Maintains records of inquiry or other information requests as needed.
  • Maintains separate “wait list” of prospective/interested residents for facility.
  • Assists with “mail outs” of inquiry letter to those who have requested information, sending out cover letter, application, brochure, and price list.
  • Demonstrates knowledge of skills necessary to identify specific resident needs and provide appropriate services.
  • Completes the social service portion of the MDS within fourteen days of admission.
  • Documents the social care component of the comprehensive care plan for each resident as identified by the interdisciplinary team within seven days of completion of MDS.
  • Notifies resident families of Medicare/Medicaid regulations, answering question and gathering information for them as needed.
  • Participates in development of interdisciplinary plan of care.
  • Meets with resident and/or family to gain information and insight to write social history and discharge plans.
  • Participates in care conference as appropriate along with others on interdisciplinary team.
  • Notifies resident/family of Medicare/Medicaid stipulations for discharge or continued stay.
  • Actively develops one-on-one relationships with residents and families as needed to be able to meet individual needs.
  • Makes determinations regarding strengths, weaknesses, and needs based on psychosocial assessments.
  • Counsels residents and families in dealing with feelings about death and dying and other emotional, mental, environmental, or physical limitations.
  • Participates in resident/family activities designed to promote social interaction, reality orientation, and intellectual stimulation.
  • Documents all resident/family interactions, which help other disciplines gain increased understanding of resident concerns/needs as appropriate.
  • Acts as a liaison with social, health, and community agencies for appropriate referrals.
  • Acts as an advocate for each resident and their rights, empowering them to be as independent as possible.
  • Provides education and resources as well as support to residents, families, and staff.
  • Acts as an information referral base for Medicare/Medicaid and insurance questions.
  • Interacts effectively with disciplinary team.
  • Actively takes a leadership role in teaching resident rights, abuse, and confidentiality to new employees at facility orientation, as required.
  • Complies with all educational in-service requirements mandated by CHI Living Communities, external accrediting, and regulatory agencies.
  • Participates and takes a leadership role as needed in various activities within the facility.
  • Oversees complaints/grievances by keeping current activity log of such and assisting with final solution/outcome.
  • Participates in Performance Improvement with active suggestion/recommendations and follow through.
  • Participates in or takes leadership role in applicable councils and support groups.
  • Assists with scheduling dental and vision doctor visits, reviewing appropriateness.
  • Participates in special projects or committee work as requested by supervisor.
  • Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.
  • Attends all mandatory in-service programs.
  • Adheres to facility policies and procedures.
  • Completes yearly online education program by deadline.
  • Attends employee meetings on a regular basis.


In addition to the essential job specific duties listed above, the Admissions Coordinator shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis. Furthermore, every employee must abide by all campus, departmental, and safety policies, rules, and regulations.

CHI Living Communities has the right to change the job specific duties and specifications required for the position of Admission Coordinator from time to time without prior notice.

Job Requirements

  • Bachelor’s Degree in Social Work, Nursing, Administration, or related field from an accredited college or university preferred.
  • Previous experience with mature adults preferred.
  • Regular attendance is an essential part of this job.
