Essential Job Specific Duties/Responsibilities

  • Demonstrates knowledge of skills necessary to identify specific resident needs and. provide appropriate services.
  • Participates in development of interdisciplinary plan of care.
  • Actively develops one-on-one relationships with residents and families as needed to be able to meet individual needs.
  • Provides education and resources, as well as support to residents, families, and staff.
  • Participates and takes a leadership role as needed in various activities.
  • Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.

Job Requirements

  • Bachelor’s Degree in Social Work from an accredited college or university preferred. 
  • Must be a Licensed Social Worker
  • Previous experience with mature adults preferred
