The Gardens at St. Elizabeth has an exciting new opportunity for an Assistant Director of Nursing for our Assisted Living and Memory Care community.

Job Responsibilities:

  • Coordinate care and services with providers, including primary care, dental, hospice, home health, and private caregivers.
  • Follow up with residents after hospitalizations, illnesses, or falls to support recovery.
  • Provide minor wound care and assist with medication administration per community guidelines.
  • Maintain accurate documentation and care records.
  • Lead health education efforts and promote fall prevention.
  • Respond to emergencies and ensure safety procedures are followed.
  • Build and maintain strong relationships with local healthcare providers and referral sources.
  • Evaluate hospital or home-based patients prior to community admission.

Job Requirements

  • Active RN license in the state of Colorado (Required)
  • Current CPR and First Aid certification (Required)
  • Experience in long-term care (Preferred)
  • Familiarity with PointClickCare (Preferred)
