In the rich Franciscan tradition of compassionate, quality care, Franciscan Care Center is a residential and rehabilitation facility for the care of the elderly and those in need of extensive rehabilitation services. We are seeking a Director of Social Services to join our team! This position requires an active LSW (Licensed Social Worker) license in the state of Ohio.

Job Responsibilities

Working with the interdisciplinary team, the Director of Social Services assesses and promotes the social and emotional health and well-being of residents and their families. The Director of Social Services provides service to adults and older adults with the goal of affirming the resident’s integrity, growth, and control over the qualities of their life.

  1. Conducts pre-admission interviews with prospective residents/guarantors and explains all services of the center.
  2. With the Director of Nursing, evaluates hospital- or home-based patients prior to transfer as a special service to hospitals and families.
  3. Explains and documents all admission forms and agreements.
  4. Coordinates admission process, interviews new residents and/or their families to determineinterests and abilities, family history, and background.
  5. Develops and implements on-going in-service education and training for staff to promote understanding of social components in long-term care.
  6. Develops and maintains relationships with outside agencies.
  7. Evaluates resident/family needs in relation to resources and refers to appropriate agency or resource.
  8. Coordinates transfer and/or continuity of care services for discharged residents.
  9. Provides emotional support to residents/families.
  10. Continually makes assessments of residents to determine progress.
  11. Keeps social records and discharge plans on all residents.
  12. Attends resident care conferences and participates in the development and update of patient plans of care.
  13. Joins community groups as appropriate.
  14. Attends seminars, workshops, mandatory in-services, and other meetings as deemed appropriate by the Administrator.
  15. Attends admissions, department leadership, and other meetings as appropriate.
  16. Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.

In addition to the essential job specific duties listed above, the Director of Social Services shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis. Furthermore, every employee must abide by all campus, departmental, and safety policies, rules, and regulations.

CHI Living Communities has the right to change the job specific duties and specifications required for the position of Director of Social Services from time to time without prior notice.

Job Requirements

  • Bachelor’s Degree in Social Work from an accredited college or university.
  • Licensed Social Worker in the state of employment.
  • Previous social services experience. 
  • Previous experience with mature adults preferred.
